Aging in Place with Confidence:

According to AARP, nearly 90% of seniors prefer to remain in their own homes as they age—and 82% would choose to do so even if they begin needing support. At CW Senior Support, we partner with seniors and their families to make that possible, offering personalized assistance and access to vital resources that support safe, independent living at home.

Here are just some of the ways we help seniors age in place with dignity and peace of mind:

  • Medication management

  • Virtual accompaniment to medical appointments

  • In-home nursing visits

  • Pet therapy for emotional support

  • Collaboration with your care team

  • Comprehensive clinical assessments

  • Consistent communication with family members

  • Acting as your Durable Healthcare Power of Attorney

  • Specialized Dementia and Alzheimer’s support

  • Fall prevention assessments and safety planning

  • Discharge planning and post-hospitalization care

  • Expert geriatric support and guidance

  • Compassionate end-of-life and hospice support

  • Telehealth and virtual consultations

  • Ongoing clinical oversight

  • Bill pay assistance

  • Fraud prevention, including mail and phone screening

  • Mail sorting and organization

  • Personalized home care referrals to match your specific needs

  • Information on community resources, such as Meals on Wheels, local Area Agencies on Aging, and other senior-focused organizations

At CW Senior Support, our goal is to empower seniors to remain in the comfort of their homes for as long as possible—safely, independently, and with confidence.
